("Checkout"), prospective investors who certify to Checkout that they are "qualified institutional buyers" (as defined in Rule 144A under the Securities Act of 1933, as amended), security analysts who certify to Checkout that they are reputable security analysts who regularly cover or intend to cover Checkout and the Discount Notes, and market makers who certify to Checkout that they are reputable market makers who regularly make or intend to make a market in the Discount Notes, may obtain access to the password protected online data system on which Checkout provides annual and quarterly reports and certain other information relating to its business, in accordance with the indenture relating to the Discount Notes, by completing the linked form (the "Certification") and sending the duly completed and signed Certification to the specified address below.
